Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Policyholder
  • Name of the Insured Person in whose relation the Claim is being lodged
  • Nature of accident / Injury
  • Name and address of the attending Medical Practitioner and Hospital
  • Date of accident
  • Any other information as requested by the Company
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

  • Death Certificate (in case of Death Claim) 
  • Disability Certificate (in case of Disability Claim) 
  • Duly completed and signed Claim Form, in original 
  • Medical Practitioner's referral letter advising Hospitalization
  • Medical Practitioner's prescription advising drugs / diagnostic tests / consultation
  • Original bills, receipts and discharge card from the Hospital / Medical Practitioner
  • First Information Report/ Final Police Report
  • Post mortem report, if available
  • Any other document as required by the Company to assess the Claim.
