Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

  • Original Death Certificate issued by Local Authority
  • Copy of Medical Cause of Death Certificate, Medical Records (Discharge / Death summary, Test results, Admission notes, Outpatient consultation notes etc)
  • Age Proof & Identification document (with Photo) of Life Assured
  • Identification document (with Photo) of Claimant
  • Proof of Bank account of Claimant (Self Attested copy of Bank Passbook / Account statement of Claimant)
  • Proof of your Current Address

Additional Document (Accidental or Unnatural Death (including Suicide) )

  • Jurisdictional Police Station Attested Copy of FIR
  • Post Mortem Report duly attested by the concerned officials.
