Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

List if Mandatory Documents required

  • Completely filled and signed Claim intimation Form
  • Original Death Certificate
  • Copy of Identification & Address proof of Nominee/Claimant
  • Copy of Bank Pass book of Nominee/Claimant along with Cancelled Cheque

In case of death due to unnatural causes including Accidents, Murder, Suicide etc, Please provide following documents.

  • Copies of First Information Report Duly attested by Police Officials.
  • Copies of Post Mortem Report Duly attested by Police Officials.
  • Copies of Panchanama Report Duly attested by Police Officials

In case the Life Insured was treated for any illness related to cause of death, Please provide following documents.

  • Discharge / Death Summary of Hospital
  • All investigation/Diagnostic Reports
  • Hospital Summary/Indoor Case Sheets
