Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Your contact details
  • Name of the Insured
  • Policy number
  • Date and time of problem
  • Nature of problem
  • Location of problem
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

  • The Policy Number

  • The preliminary medical report describing the nature and extent of all injuries or Sicknesses, and providing a precise diagnosis

  • All invoices, bills, prescriptions, Hospital certificates which will permit the Company to accurately determine the total amount of Medical Expenses  (if applicable) incurred by the Insured Person

  • In the case where another party was involved (e.g. a car collision), the names, contact details and if possible insurance details of the other party

  • In the case of death, an official death certificate, succession certificate pursuant to the Indian Succession Act 1925, as amended, and any other legal documents establishing the identity of any and all beneficiaries

  • Proof of age, where applicable

  • Such other information as the Company may require to handle the claim.

  • Any police reports concerning the Accident

  • The date a Physician was seen due to the Bodily Injury

If a Sickness

  • The date symptoms of the Sickness began
