Duly completed claim form

  • Attending Doctors Report 
  • Original policy for cancellation
  • Original Death Certificate
  • Original / attested Post Mortem / Coroners Report, where applicable
  • Attested copy of FIR / Panchnama 
  • Police Inquest report, where applicable
  • Any other document required by the company Post mortem report if necessary, be furnished within the space of fourteen days after demand in writing  

Permanent Total Disablement/ Permanent Partial Disablement/ Temporary Total disablement
