Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Insured Detail
  • Claimant’s/Deceased’s Details
  • Details of Accident
  • Details of Injuries Sustained
  • Nature of disablement
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

Death claims:

  • Death Certificate 
  • Post mortem report. 
  • Chemical analysis report / viscera report. 
  • Inquest Panchanama report issued by the Police. 
  • First information report. 
  • Admission/Discharge/Death summary (if applicable)  
  • English translation of vernacular documents  
  • Legal Heir Certificate/ Succession Certificate where nomination has not been made 

Disablement claims

  • Medical Certificate forming part of the claim form.
  • Investigation reports (Laboratory tests, X- rays and reports essential for 
  • Confirmation of the injury such as MRI report CAT Scan etc.) 
  • First Information Report where applicable
  • Medical bills and cash receipts. 
  • Admission/ Discharge summary.
