Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Your contact numbers
  • Policy number
  • Name of insured person who is injured
  • Date and time of accident
  • Location of loss
  • Brief description on how the accident took place
  • Extent of loss
  • Place and contact details of the Insured Person.
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

  • Original Death Certificate.
  • Post Mortem Report
  • Inquest report. 
  • Accident report. 
  • FIR/MLC copy. 
  • Hospital records.
  • News Paper cuttings if any and any other relevant records. 
  • Chemical Analysis Report if available. 
  • English Translation of vernacular documents.
  • Succession Order/legal heir certificate/legal documents to establish identification of legal heir in the absence of nomination under the policy.

  • Disability Certificate issued by attending physician.
  • Accident report.
  • FIR/MLC copy.
