If the policyholder wants to surrender his/her life insurance policy or make a partial withdrawal from the policy fund, the following procedures need to be followed:
Surrender Policy
Reason(s) for policy surrender and the surrender form needs to be submitted at the nearest Aviva India Life Insurance branch, along with the following documents:
Basic Mandatory Documents:
- Payout form
- Original policy documents
- A self-attested ID proof of the policyholder (PAN Card, Aadhaar Card, Passport, Driving License, Voters ID)
- Recent colored photograph
- Canceled cheque of the A/c where the NEFT is required
- Self-attested bank statement/ passbook copy showing the premium remittance or last six months bank statement.
Additional Documents:
- If the policyholder's address endorsement is done within a year or is requested along with the policy surrender/partial withdraw/free-look period then a self-attested copy of documents of the current communication address of the policyholder are required.
- If original/duplicate policy documents are not submitted then a self-attested copy of police complaint/FIR or indemnity bond (on Rs 100 stamp paper) is required.
Additional Documents Required for Money Transfer in an NRE Account:
- Canceled cheque from NRE A/c
- NRE bank statement reflecting any premiums paid from the NRE account/copy of bank passport of NRE A/c from which premiums are remitted.
Reinstatement of Policy
In case the policy lapses and the policyholder wants to reinstate the same (following the policy terms & conditions) then the following steps have to be followed:
- To revive a lapsed policy, the policyholder will have to fill an application that declares good health
- The policyholder has to provide complete details along with related documents.
- Both the policyholder and the life insured have to sign the form. If the policyholder is different from the life insured.
- Complete and submit the form to the nearest Aviva India Life Insurance branch.
Partial Withdrawal
Reason(s) for the partial withdrawal and the pay-out form needs to be submitted at the nearest Aviva india life insurance branch?, along with the following documents:
- Original policy documents
- Copy of CI or YPD submitted for the withdrawal request (carry original documents for verification in the branch)
- Canceled cheque of the insured's name
- In case the canceled cheque does not have a pre-printed name and account number or a new account is mentioned on the cheque, then passbook copy/bank statement having the pre-printed name and the account number is required
- ID proof (PAN Card, Aadhaar Card, Passport, Driving License, Voters ID)
- Pay-out form
- Latest contact details
- NRE bank statement reflecting any premiums paid from the NRE account
