How to intimate Life claim with AEGON Life Insurance

Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/Event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

Points to Remember

  • A death claim needs to intimated as soon as possible after the occurence of the event to the insurer.  
  • Please ensure timely submission of claim file for speedy decision.
  • Probability of investigation is less if the policy is more than 3 years old.

List of Documents

Mandatory Documents for all type of claims:-

  •  Death certificate (original or self-attested copy) issued by municipal  corporation under section 12/17
  •  One passport size color photo of the claimant
  •  Passbook copy and Cancelled cheque stating the claimant's (Nominee) or life  assured account number (as applicable)
  •  ID and residential proof of the claimant.
  •  Relationship proof (wherever applicable)

  •    ALIC Attendant physician statement for death claim
  •    ALIC Hospital treatment statement for death claim

Accidental Death Claim / Suicide Death Claim:-

  •   Post mortem report - duly filled
  •   Police inquest report
  •   Panchanama / First Investigation Report
  •   Copy of Driving License of the Insured (if died due to road accident)

Accidental Disability / Dismemberment Claim:-

  •   ALIC Disability / Dismemberment Claim Application Form
  •   ALIC Attending Physician Statement
  •   ALIC Hospital Treatment Certificate
  •   Police Inquest Report
  •   Panchanama
  •   First Investigation Report
  •   Copy of Driving License of the Insured (if died due to road accident)

Critical Illness Claim/ i Cancer Claim:-

  •  ALIC Critical Illness Claim Application Form
  •  ALIC Critical Illness Attending Physician Statement
  •  ALIC Critical Illness Hospital Treatment Certificate
  •  All Medical reports from the first date of diagnosis to the last treatment received date
