How to intimate Life claim with Aviva Life Insurance

Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

Points to Remember

  • A death claim needs to be intimated as soon as possible after the occurence of the event to the insurer.  
  • Please ensure timely submission of claim file for speedy decision.
  • Probability of investigation is less if the policy is more than 3 years old.

  • Copy of death certificate issued by local authority

  • Medical Records (admission notes, discharge summary, test reports etc)
  • Last Medical Attendance Report ( LMA)
  • Post Mortem Report ,if post mortem done
  • FIR / Police Report if case filed with Police
  • Panchnama / Inquest Report if case filed with Police
  • Saving Bank Account Number
  • Newspaper cuttings
  • Contact number of Claimant
  • Existence Certificate
  • PAN card of payee
