Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

List of Documents

  • Original Policy Document, Claimant Photo Identity & Address Proof
  • Copy of Death Certificate
  • Copy of Medico-Legal cause of Death Certificate
  • Medical Records (Indoor Case papers, Discharge/Death Summary, Test Reports etc) on the Life Assured being hospitalized
  • Hospital Certificate (Death/Rider), if the Life Assured was hospitalized
  • Treating Doctor Certificate
  • Employer Certificate for Salaried policyholder
  • Post Mortem Report (if conducted)
  • FIR/ Police Report/ Panchnama / Inquest report (if case filed with Police)
  • Copy of Driving License if Life Assured was driving the vehicle (Applicable if "Accidental Death Benefit or Accidental and Total Permanent Disability Rider" is opted)
