Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

Death (all causes of death)

  • Claim Forms
  • Part I : Application Form for Death Claim (Claimant’s Statement)
  • Part II : Physician’s Statement
  • Death certificate issued by a local government body like municipal corporation/village panchayat
  • Medical cause of death certificate issued by attending physician
  • Attested true copy of indoor case papers of the hospital(s)
  • Post mortem (autopsy report) & chemical viscera report – if performed
  • Advance discharge voucher from Group Administrator
  • KYC of claimant along with cancelled cheque (bearing the name of claimant) or passbook of claimant in case the payment is to be made in name of claimant. Declaration from master policy holder is also required on letter head of master policy holder.

If death due to accident(submit in addition to the above)

  • All police reports/first information & final investigation report
  • Proof of accident – panchnama/ inquest panchnama, etc.
  • Newspaper cutting/photographs of the accident – if available.
