How to intimate Life claim with Shriram Life Insurance

Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Policy Number
  • Name of the Claimant
  • Name of the Life Assured
  • Date of death / event of the Life Assured
  • Cause of Death/event
  • Contact Details
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

Points to Remember

  • A death claim needs to intimated as soon as possible after the occurrence of the event to the insurer.  
  • Please ensure timely submission of claim file for speedy decision.
  • Probability of investigation is less if the policy is more than 3 years old.

Mandatory Documents for registering a claim

  • Duly filled Claim Forms
  • Original Policy Documents
  • Original Death Certificate issued by municipality or Equivalent
  • Claimant’s photograph, current address proof and photo id proof. 

Supporting Documents

  • Copy of attested Bank Passbook/Bank statement
  • Medical Certificate stating the cause of death
  • Medical records like consultation notes, treatment records, hospital indoor case sheets, Discharge/Death summary etc.
  • FIR Copy/Inquest report/Panchanama
  • Post Mortem Copy/Viscera report (Chemical Analysis Report)

Additional Information:

  • All submitted documents should be original or photocopies attested by a gazetted officer, magistrate or Sarpanch, Tahsildar or Police.
  • Based on the circumstances of the claim Shriram Life Insurance reserves the right to call for additional documents/information.
  • The company also reserves its right to conduct inquiry/independent inquiry/investigation to prove the authenticity of any claim where it is considered necessary.
  • Legal entitlement in case there is no nominee. (Legal heir Certificate)
