Details required for Registration/ Intimation of Claim

To Register a claim through the above contact points, the following information has to be provided to the customer support executive:-
  • Your Contact numbers
  • Policy Number
  • Name of Insured
  • Date & Time of Loss
  • Location of Loss
  • Nature of Loss
  • Place & contact details of the person at the Loss Location.
Once the claim is registered, a Claim Reference/Intimation Number will be generated and has to be preserved for further correspondence.

  • Every claim should be intimated & file to be submitted within time, as non adherence can lead to delay in claim settlement.
  • The claim is decisioned only when all required & necessary documents are submitted.

List of Documents

  • Our claim form, duly completed and signed for on behalf of the Insured Person.
  • Original Bills & Receipts (Only in Reimbursement benefit) otherwise Photocopy of Bills and any attachments thereto like receipts or prescriptions in support of treatment taken 
  • All reports, including but not limited to all medical reports, case histories, investigation reports, treatment papers, discharge summaries. 
  • A precise diagnosis of the treatment for which a claim is made. 
  • A detailed list of the individual medical services and treatments provided and a unit price for each. 
  • Prescriptions that name the Insured Person and in the case of drugs: the drugs prescribed, their price and a receipt for payment. Prescriptions must be submitted with the corresponding Doctor’s invoice.
  • Original Death Certificate 
  • Original/ Attested Post Mortem Report, if conducted.
  • Attested copy of FIR, Spot Panchanama & Police Inquest report, where applicable.
